Midface Filler

A filler treatment that restores midface volume loss and refreshes a tired appearance.

The mid-face is the region at the centre of facial expression, extending from below the cheekbone to the side of the nose and from under the eye to the upper lip. This area often carries the earliest and most visible signs of ageing: with volume loss the cheeks flatten, the under-eye groove deepens and the face takes on a tired look. Mid-face filler aims to give the face a rested, natural and balanced appearance by addressing these losses.

Why does the mid-face lose volume?

In a young face the mid region is full and directed upward. With age the deep fat pads shrink, bone support recedes and the tissues slide downward. As a result, cheek volume decreases, the transition between the under-eye and the cheek becomes pronounced (the tear trough) and the nasolabial lines deepen. The face begins to look tired and drooping even when not smiling.

Mid-face filler cannot reverse this process; but by restoring lost volume it makes the face look younger and more rested. Most of the time it requires a plan that treats not a single region but the mid-face as a whole.

How is it applied?

Before the application, the face is assessed both at rest and while making expressions, because the mid-face moves markedly during smiling. A topical anaesthetic cream is applied for comfort. The filler is mostly injected with a blunt-tipped cannula in a deep plane and little by little; this technique both provides natural support and reduces the risk of bruising.

The procedure usually takes 20–30 minutes. The result is largely seen immediately; after the mild swelling of the first few days subsides, the final, natural appearance emerges.

Longevity and a natural result

The longevity of mid-face filler varies, depending on the product used and the person's metabolism, on average between 12–18 months. Since the support of the region naturally decreases over time, renewal at certain intervals may be preferred to maintain the result.

The mid-face is like the ‘roof’ of the face; when it is supported correctly, all the lines beneath it also look more rested.

The key to a natural result is to be measured. Excessive volume can give the face an artificial and ‘puffy’ look. For this reason, starting with a small amount and adding when necessary is both a safer and aesthetically more correct approach.

After the application and safety

For a few days after the procedure, it is recommended not to massage the area firmly and to avoid lying face-down and very hot environments. Mild swelling and rarely small bruises may occur; these pass in a short time. In cases of pregnancy, breastfeeding, an active skin infection and an allergy to the filler content, the application is not performed.

Its relationship with the under-eye and tear trough

Mid-face volume directly affects the appearance of the under-eye region. When cheek support decreases, a shadow and depression (the tear trough) become pronounced between the under-eye and the cheek; this gives the face a tired expression. In many cases the cause of under-eye darkness is not the under-eye directly, but the decrease in the mid-face support beneath it.

For this reason, mid-face filler sometimes indirectly improves under-eye hollowness. Which region is prioritised is planned according to the person's anatomy; in some cases the mid-face and under-eye are addressed together.

Is the result seen immediately? Largely yes, but for the first few days the area may look fuller than it is because of mild swelling. The final, natural appearance emerges after the swelling subsides; this is why it is important not to evaluate the result from the first day.

Is a cannula or a needle used? In the mid-face a blunt-tipped cannula is often preferred; this method both provides natural support and reduces the risk of vessel injury and bruising. The choice of method is made according to the region and the physician's assessment.

In which situations is filler alone not enough?

Mid-face filler is effective for addressing volume loss; however, in cases of advanced skin sagging and laxity it may not be sufficient on its own. In such cases the filler can be evaluated together with methods that provide skin tightening. The aim is to support skin quality while replacing the volume.

In some people the problem is not only volume loss but the downward slide of the tissues. Correct planning makes this distinction and determines the most suitable approach. For this reason, before the question ‘how much filler’, the question ‘which region, and why’ is answered.

In situations such as pregnancy, breastfeeding, an active skin infection and an allergy to the filler content, the application is not performed. A detailed assessment before the application safeguards both safety and the quality of the result.
